The Community Navigator was developed to help in navigation of existing services and to respond to the gaps in service delivery. The Community Navigator acts as a bridge from social isolation and segregation towards civic involvement and social engagement.

Peer Support and Guidance
We bring families together to share experiences and learn from each other.
Establishing Existing Relationships
We establish what the existing relationships in your life are. These relationships range from exchange to participation, friendship and intimacy.
Building a Community Presence
We work with you to develop new relationships and identify community spaces for you to explore.
Building a Support Network
We work with you to establish trusting and committed relationships and support you in inviting these relationships into a support network.
Developing a person-centered plan helps what once felt impossible seem possible again. Through guided conversation and deep listening we can develop a clear vision for the future. Where each person's unique gifts and abilities guide the way to a life that is rich in meaning and purpose.
